Anishnawbe Health Toronto is a fully accredited health centre, whose model of health care is based on Indigenous culture and traditions. AHT is the only facility in Toronto that cares for Indigenous clients with both western and traditional approaches to health care.

Youth Outreach Worker (YOW) Services

“YOW services includes outreach, short-term support, referrals, and consultation that utilizes evidence-informed outreach models based on the Stages of Change theory. This service model is a method of categorizing and supporting an individual’s readiness for change. The emphasis is on bridging youth to AHT services and other external programs, providing accompaniments and support to ensure successful connections to programs, workers and other service providers.”
